Maximal oxygen uptake (VO2 max) is measured commonly in fitness tests to determine cardiovascular and muscular fitness. Physical work capacity, also assessed by VO2 max, is a measure of the maximum work capacity of the heart-lung system. Several studies suggest that a decline in VO2 max and physical work capacity among men is associated with a change in mortality, although it has been suggested that changes in blood pressure or in the use of medication may explain the association. The purpose of this study was to investigate the change in VO2 max and physical work capacity in relation to the change in survival and to examine whether changes in blood pressure and medication use account for the observed association between change in VO2 max and survival. We studied men born in 1918-23 and followed up for 7 years from the Midlife in the United States study who had a VO2 max measured at baseline and at 7-year follow-up and were free of coronary heart disease and diabetes at baseline (n = 3870). We examined the association between change in VO2 max (5.0 vs 9.1 mL/kg per minute) and change in work capacity (mL/kg per minute) (exercise stress testing), and survival with Cox proportional hazards regression. During 7-year follow-up, 88 men died and median survival was 16.0 (95% confidence interval, 14.1 to 17.9) years. Compared with a decline in physical work capacity, an increase in VO2 max was significantly associated with a greater risk of death (adjusted hazard ratio, 1.37; 95% confidence interval, 1.05 to 1.79; P =.02).
